Following the transfer of ownership from Turbine to SSG, DDO no longer has an updated list of Official Known Issues. The following stands as an archive of the final official list, made in late 2012.
If you can verify that the subject still exists, please edit this page and remove this template.
Below is a copy of the Official known issues from the official DDO Forums. Maintained here as a reference for when the forums are offline. Please do not edit mistakes in this list (except to update the copy to match the forums), and rather add them to the player notes section.
Below the official known issues are features that are Working as Intended but may cause confusion. Should you come across any of these issues, There is no need to submit a bug report about these issues unless otherwise noted.
Current Official Known Issues (Last updated by Turbine 11-12-2012)
Of Special Note
NEW: During a reincarnation, a display error can cause the XP bar to not properly show character XP even though the player has XP and can level up with the Reincarnation trainer.
Old and completed in-game help requests will push an inaccurate ticket update notice when you log into a character. This issue is cosmetic and does not mean that you have any open issues with customer service. Removing the ticket status will still result in the same issue the next time you log into the game. We’re working to resolve this issue and appreciate your patience.
Windows users with desktop shortcuts pointed to "dndclient.exe" may experience a launcher error. Ensure that your shortcuts are pointed to "turbinelauncher.exe" instead.
Some Intel users may find that when DirectX 10 is enabled, only their UI is visible and the rest of their screen is black. If this occurs, we recommend you disable DX10, enable DX9, and restart the game client.
If your quest fails to complete, logging out and back in can re-update your quest status to grant the completion. Make sure you have someone in the quest to keep the instance active for you while attempting this work-around. Solo players please note: logging out and back in may help, but only if your instance does not reset in the process. If the instance resets you will need to redo the quest.
Players have reported that they sometimes have trouble climbing up long ladders due to hitching or becoming caught in the middle of the ladder. Many of these "sticky" ladders have been fixed, but there may be other locations. If you come across a sticky ladder, please submit a bug report from the location of the ladder.
Classes
Barbarian paths are not bestowing their passive dodge bonuses following a level increase until a character log out and back in.
Crafting
Runestone and ammunition recipes lack full descriptions.
Some ingredient list icons do not have backgrounds in the recipe ingredient list.
Changing sort options in a Cannith Crafting device does not reset the device recipe list's page number.
Favor-based crafting ingredients need to be manually dragged into the recipe ingredient bar.
Players can disjunct an item multiple times in a row, even though the item has already been prepared for crafting using a Dust of Disjunction.
Shards of Bashing are only listed as "Shard" when crafted.
The Shard of Masterful Craftsmanship is not being placed into the proper slot in a crafting device's ingredients UI unless greater and lesser essences are placed into the UI first.
It is possible to deconstruct, but not craft, converted collars made from handwraps.
Crafting essences are not appearing in end reward lists.
Items in ingredients bags are sometimes "invisible" when logging back in. This is a display issue only; the items remain in ingredients bags.
Immediately attempting to disjunct an item prepared for crafting using a Dust of Disjunction will consume another Dust of Disjunction, even though the item is already prepared for crafting.
The "Craft" button is invisible until a recipe is selected from a crafting device UI panel.
Information about the various crafting schools does not display when the mouse pointer hovers over the name of the school. This information displays when the pointer hovers over the level bar.
Ingredient bag tooltips incorrectly appear underneath the ingredient bag UI.
The "Clear" button sometimes stops working.
Ingredients bags that contained Everbright Shards (Shield) continue to display unusable versions of these shards, even though the shards have been removed from the game.
When a player fails to craft a recipe, the crafting device does not animate or make a noise. An alert message is properly displayed, however.
The Shard of Tendon Slice fails to give the tendon slice mutation to an off-hand item, but will overwrite existing suffixes and consume the shard.
Reconstruction I shards and recipes do not mention that they affect Repair spells.
The tooltip for Equipment Disjunction simply states "item."
It is possible to apply Incite +20% to a bow, which increases threat generation to main or offhand melee attacks. Since this is not possible with a bow, the mutation has no effect.
Some NPCs have typos in their dialog.
Items fixed or modified using the Stone of Change or other Eldritch Devices will sometimes not display the updated effects. This is a display bug only. Removing, and then re-equipping, the item will show the updates to the item.
Challenges
Several Dryads have "DNT" text in their mapnote.
When visiting the Challenge Trader in House Cannith, the tooltip for Calomel's Repeating Light Crossbow shows the tooltip for the heavy crossbow.
There is an extra mapnote for the Extraplanar Mining: Labor Shortage challenge.
Life Replenish is described as a regeneration effect when it instead heals in one burst of healing.
Challenge Loot
The effect of Staggering Blow is not working properly.
Creature Companions
Creature companions rapidly teleport when their owners are swimming, flying, or using an elevator platform.
DDO Store

You must close and reopen the DDO Store to purchase each upgrade of the Shared Bank: Platinum Vault.
The following Hirelings are incorrectly labeled in the DDO Store:
Tome is a Wizard, not a Sorcerer
Gorgal Orc-hewer is a Barbarian, not a Fighter
Selecting Paypal as a payment method can cause the game to close.
Hard and Elite difficulty unlocks are not filtered out of the DDO Store for VIP players.
After playing for extended amounts of time in the game, your DDO Store session may time out when you attempt to open the Store. To correct this, press the home button (the house shaped icon in the upper left of the Store window) to reload the Store.
Players who purchase the Maple Airship Interior Stain receive a white version instead.
"Continue shopping" store link takes you to the homepage
If you meet the pre-requisites to see an item in the Store (such as having a character ability or purchasing a prerequisite item), you may need to reload the Store for the item to appear. Press the home button (the house shaped icon in the upper left of the Store window) to reload the Store.
Returning players who are not receiving their favor reward Turbine Points may be able to correct the issue by logging out and back into the game to receive residual Points. Refreshing the Store using the home (the house icon) button may also correct the issue.
Shared Bank Space will not filter out once purchased, allowing multiple purchases without granting multiple Shared Banks (there is no such thing as multiple Shared Banks).
Enhancements
NEW: Pale Master's "Shroud of the Vampire" does not deactivate properly.
Unyielding Sovereignty is interruptable, and goes on a cooldown timer if interrupted.
None of the homunculus enhancements list their requirements.

Wizard Archmage: Protection from Energy is using an octagonal rather than square icon.
Enhancement, Paladin, Divine Might: The charisma requirement for Divine Might is listed in the description but does not show in the requirements section.

Non-bards currently receive more songs per rest than intended when twisting active Fatesinger abilities.
It is possible to override Energy Sheath with lower-powered buffs of the same type.
Events
Euphonia's Challenge: Treasure of the Crystal Cove
Level 1 pirate hat upgrades can be applied multiple times to no effect.
Hirelings do not share the gems they receive from killing monsters in Crystal Cove. Players can ensure they get their fair share of the loot by delivering the killing blow.
Torches can be placed in a location that puts the torch several feet into the air, causing confusion for kobold miners. Players who experience this issue should pick the torch up, and move it to a different spot.
*Double-clicking on the Pirate Port from the hotbar (or using two in short succession) can crash the client.
Casting any type of crowd control while dropping a torch, barrel, or healing torch makes the item disappear, and not be able to be re-acquired.
Torches can get stuck in walls, causing kobold pathing problems.
A torch can sometimes appear not to connect to a line of torches until an additional torch is placed. This is an occasional graphical glitch, but does not impact the actual functionality of the torch.
Tumbling while placing a torch does not place the torch, and removes the torch from inventory.
The drop rate for requisition items received through killing boss monsters in Crystal Cove is lower than intended. The requisition item prices have been temporarily lowered in the DDO Store.

Level 1 arcane hirelings do not cast all of their available spells.

Modules with guild augment slots are not able to be equipped by Iron Defenders.

NEW: - The "Claim Destiny" button will not appear unless you own Epic Destinies. Epic Destinies are not included as part of a VIP subscription.
Many crafted items minimum levels now match their randomly-generated treasure counterparts.
Due to the new UI layout saving option, UI settings have been returned to the default positions. You may arrange your UI to your preferred layout and then save it by typing “/ui layout save [filename]” in the chat box. Replace "[filename]" with an easy to remember name for that layout. This layout is stored locally, and the layout files can be exported to other computers. You will only need to do this once for each character that you prefer a custom layout for.
The DDO Unlimited key mapping will over-ride existing Stormreach key maps. Make a copy of your 'ddo.keymap' file before using DDO Unlimited to retain your old settings.
If you are downgrading from a VIP account, any characters of a premium race and/or class that you have not purchased with Turbine Points or unlocked with favor must be purchased/unlocked before you will be able to access the character as a free or premium player. Example: If you downgrade and select a warforged monk, you will need to purchase monk and warforged before you'll be able to play the character.
Remember that if you take Jeets offer to skip the tutorial you will be advanced ahead in the storyline of Korthos. In order to group with friends who have not yet unlocked the ability to skip the tutorial, decline Jeets offer to skip ahead and complete the grotto tutorial instance. This will allow you to enter the snowy version of Korthos village to group with other players in that part of the story!
DDO Store
Once Favored Soul or Drow is unlocked with favor points, it is still accessible in the DDO Store. This is by design, as purchase in the store will grant Favored Soul or Drow access to your account for all servers, while favor unlocking is per-server only.
Viewing the Store before you login a character will only display account level character purchase items. To see a full list of items available to your character, log in the character.
The DDO Store's automatic filtering does not recognize feats or enhancements that allow you to overcome prerequisites for some items. This can result in the Store blocking some items from appearing in your catalog because the store doesn't think you are able to use the items. To see and be able to purchase these items, turn off filtering.
Hirelings
Hirelings will path to monsters across water if they think they can jump across. They aren't very good swimmers though, and if they don't make the jump, you will have to recall them to you.
In Game Vendors
When selling to pawnbrokers, remember that unlike Tavern-keepers and general vendors, other players can purchase an item you've sold to the NPC. Additionally, it is intended that items sold to pawnbrokers will not appear in your buyback list. Instead the item will appear for purchase in the sell tab of the pawnbroker's list.
Tad Hardcastle spent so much on his new acrobats outfit that he has tried to cut some corners on his donation gifts. The Greater Tumble spell he casts will only allow players to tumble if they have more than 0 ranks in tumble.
Items
Airship beacons cannot be used in Delver's Canteen.
Skills, Feats, & Abilities
The minimum ranks in the Perform skill required to use Bard songs are based on the 'ranks' taken, not the modified skill total. This is by design.
Quests
Diplomatic Impunity
Root walls cannot be broken with bows.
Player Notes
Player notes last updated July 14, 2021.
Of very special note
The Bug Reporting tool:
The ingame bug reporting tool has known to be extremely buggy and unreliable for the last 6 years. The best place to submit bugs if you truly want them to be seen by the devs is the official forums. The best place to submit them if you want them to be seen by the QA team (who may or may not forward them to the devs) is the Bug Report form on the official forums.
Known issues that have already been fixed but not removed from the official list yet
Cyclonic Blast damage numbers from the spell do not appear in the combat log.
Display fine.
Reconstruction I shards and recipes do not mention that they affect Repair spells.
Confirmed fixed already.
No resurrection shrine appears at the end of the fight against Velah.
The resurrection shrine does appear.
Known issues that simply make no logical sense
Windows users with desktop shortcuts pointed to "dndclient.exe" may experience a launcher error. Ensure that your shortcuts are pointed to "turbinelauncher.exe" instead.
Running dndclient.exe directly has never worked, the launcher has always been required as it passes the login credientials to that client exe. Perhaps it refers to dndlauncher.exe, but tests show that file works the same as turbinelauncher.exe, so this note is rather pointless.
Khyber and Siberys dragonshard fragments have weight, even when placed inside of ingredient bags.
In a previous patch they wanted to make all ingredients weight either zero or less. And again in several forum posts. These can be several hunderd pounds now. Listed as intended, yet it's obviously not intended.
Hard and Elite difficulty unlocks are not filtered out of the DDO Store for VIP players.
This makes no sense because VIP players deserve the right to be able to purchase these. Especially if they go on sale and the player knows their subscription will run out soon.
The spell point cost for Grease is incorrect.
This spells cost was already recently changed. It's now 6SP, which is appropriate since it's a very weak low level spell that does not scale well at higher levels. Likely this was already fixed and they forgot to remove the note.
The drop rate for requisition items received through killing boss monsters in Crystal Cove is lower than intended. The requisition item prices have been temporarily lowered in the DDO Store.
The current drop rate is nearly 100% for these items. Doesn't make sense that they would want it any higher. Likely it was up'd already and this note was not removed.
Some NPCs have typos in their dialog.
Too vague to be any use to players. Should we never bug report typo's because you know some exist?
Life Replenish is described as a regeneration effect when it instead heals in one burst of healing.
Nope. It works as describe, it's a healing over time effect, heals quite a lot of hp every 2 seconds.
Known issues that are a vague, which we can clarify
Superior spell potions are not applying the correct damage increase.
This refers to the fact some only do +50% dmg, when they should be increasing it by +75%. Examples are the glaciation potions from the risia festival event.
Warforged are missing their death animations.
They're only missing a few death animations, some display fine.
Some of the Warforged in Secrets of the Artificers are prone to this; might this be what the note is referring to?
Known issues that have been in the official list for over 5 years
The following potions can not be used while raged. They will be updated in the future. Neutralize Poison, Remove Curse, Remove Disease, Remove Fear.
The future.. What is the future in Turbine time? 10 years?
To clarify this note: You can't use any "clickies" while raged, and these potions are set to function as clickies. They actually did fix one potion to work as a clicky while raged: Lesser Restore. This was fixed some years ago. When/if the others will be fixed is unknown. There is a workaround - you can use guild vendor potions as they are not clickies (but only if you're in a sufficiently high level guild).
If you hit both levers lowering the bridges near the same time, the scripting breaks and neither bridge lowers, making the quest unable to be completed.
However, the game is not checking correctly if higher value ability tomes have already been consumed when the character's level is still too low to provide the total bonus obtained. (For example, a character at level 3 that has consumed a +5 Tome of Strength and currently has a +3 bonus from the tome due to their level will still be able to consume a +4 Tome of Strength.) In that case, the lower value tome is consumed and lost.
Supreme Tomes become unavailable in the DDO Store for a character that has already used an ability tome of equal or greater value for any ability, even if they have lower values in other abilities. To bypass this, you can use another character to purchase the tome and then transfer it to the intended character via the Shared Account Bank.
There might be a bug that allows some characters take AotS despite having incompatible alignment. However, such characters cannot take the True Patron: Fiend feat.
When the Rakshasa is defeated, both portcullises open (and both levers lock!), allowing you to return to the central area. But, after the Glyph is used but before the Rakshasa is defeated, the levers can still be pulled, starting the timer to close them again, likely locking everyone behind that gate, and then only Dimensional Door can get you out without re-entry. So do not use any levers in the Monkey wing after someone starts the fight, the first lever might lock you in and the second lever triggers a disjunction effect in the Rakshasa room.
Although the "/alias shortcut ;alias shortcutkey" command is currently bugged, it can be used to add additional toolbars in the 11+ range. By typing "/alias shortcut ;$$$ ###", where $$$ is a defined alias, and ### is a three digit number in excess of 100, an additional toolbar can be created. These toolbars will appear when the character is next logged in to the server. Please note that as this is an undocumented effect, some experimentation is still required, and the effect may change at any time.
Despite the in-game description, the bypass will continue to work in any form as long as the spell was initially cast as one of the listed creature types. This means any druid can cast this spell as an animal form (such as Wolf), change forms (such as not wildshaped), and still benefit from the effects of the spell.
There is a bug that benefits parties associated with the defeat of +Lichabel, and the amount of Party Favor that you get, and it can be checked out here: Nerfed party favor rate?ARCHIVE ME!
Dodge Bonus' description claims to grant an Insight bonus, but it's a normal Enhancement bonus. Inherent Electricity Resistance claims to provide an Insight bonus but it's a Competence bonus instead.
If you jump or fall down to a different surface while sneaking, your speed may reset to a slower value until you exit and re-enter sneak mode, or switch weapons.
Appears to always spawn with its Normal statline. This and the fact it is missing from the Monster Manual suggest it is summoned by the Rusty Iron Kitchen Key.
Baktor gets stuck in a teleport loop and becomes extremely hard to damage. Luckily there are workarounds:
Gather you entire party on the crate in the center, and cast mass invisibility. Baktor will become confused trying to find you, and reset his AI, fixing the bug. Regular invisibility cast on everyone one at a time can work also, just faster and easier with mass.
Alternatively if you have no access to invisibility you'll just have to beat him down with the bug, Blade barrier is rather effective when he's in this state.
this feat disappears upon Epic Reincarnation with no way to get it back except by heroic reincarnation and favor reset. No ETA on bugfix (See: DDO ForumsARCHIVE ME!)
On Epic, this monster has incorrectly scaling CRs if confronted in the Private Rooms. On Epic Reaper, Bolal is just CR 8 if confronted in the Private Rooms.
Unlike the multivial spells, this ability requires a target. (You can't just throw this ability whenever you want; there needs to be an enemy in order to be allowed to use this ability.)
The exit portal in CitW can fail to spawn. If Ana's babysitter has a long-jump capability, it may be possible to float to a nearby island on the main map and cause her to teleport, otherwise the raid will be incompletable, since Ana is stuck on the island.DDO ForumsARCHIVE ME!
Chain Missiles shoots 1 large highly damaging magic missile at your target, which deals twice the force damage of a standard magic missile (2d2+6). Then 10 standard 1d2+3 missiles explode outwards striking randomly at nearby targets. You seem to get all 10 missiles regardless of your caster level. The magic missile damage is verified as 1d2+3.
Update 48 saw many augments revamped and made more powerful to match Cannith Crafting values at the same level; however, the text in the barter interface was not updated to match. Tooltips will show the correct, higher, values.
Inscribing this spell from a scroll, instead of learning it during leveling, results in the caster knowing and casting the scroll version (produces only +1 bolts) instead of the Artificer spell version version (bolts scale with Arti Level, from +1 to +6).sourceARCHIVE ME!
Level 20 version has worse version of Nightmares than Level 15 version. The descriptions on both items are wrong, as Greater Nightmares works up to 1500 hp and Improved Nightmares up to 1000 hp.
Can be accidentally attacked while acting as an NPC, in which case it will turn hostile. The monster will still be called Phiarlan Guard, but will count as Crazed Phiarlan Guard in the Monster Manual.
Be careful with the timing of lowering the drawbridge. Make sure garko is not at the beginning of the drawbridge when you lower it, or he may become stuck.
This spell receives full benefit from spell power granted by metamagics (tested with Maximize), but only 50% benefit from other sources of spell power. (E.g. casting this spell Maximized with 400 positive spell power on your character sheet will have an effective spell power of 400 * 50% + 150 = 350.)
Some items will not appear for a character even if the ML is met. Example: Ruby Eye augments are ML1 and will not appear unless the character is at or near the level cap.
If the fight ends with an active DoT on Tidus, he may attack the party again. In this state he cannot be subdued, therefore making his objective and thus the quest impossible to complete. This can be avoided by leaving at least one of his bodyguards in the fight until he himself is defeated.
On Epic, this monster has incorrectly scaling CRs and is red-named if confronted in the Library. On Epic Reaper, Dariel is orange-named and just CR 6 if confronted in the Library.
Death Knight's Undead traits are partially broken so they are a sort of mix of human and undead. While they have the 100% fortification undead possess, they are missing many other traits.. Most notably they can be healed by players AOE healing. Also their Improved Sunder does not reduce fortification like it should.
Because of the way this quest changes, all the mobs are considered their equivalents on Heroic Normal for the Monster Manual (MM). Monsters listed on Heroic Hard and higher difficulties, if not found in other quests, will not be added to the MM count (with the notable exception of Baktor the Horned Devil on Epic.) On the other hand, this means you can see HP totals for monsters on higher difficulties if you have the HP view reward for their Heroic Normal counterpart – for example, since the 5th wave of Heroic Normal is composed exclusively of troglodytes, if you have Troglodyte Mastery, you can see the HP of every monster in the 5th wave of every difficulty.

If, after completing all 5 quests, you close the arc reward list offered by Haden d'Jorasco without choosing something, that list is not offered again and you lose that reward.
Currently, this spell appears to deal significantly more (at least twice as much) damage on a successful save than the official description indicates it should (1d3+3).
The tooltip still names any religious class as requirement. This is not possible, since the Follower of the Path of Light feat is exclusively for Sacred Fists.
The Update 66 Release Notes for Dragon Lord claim the class is meant to have a Good Will Save progression (using the same numbers as Fort Save in the below table); the archetype still uses Fighter's Poor progression for this save (same numbers as Reflex save.) It is unclear if this is a bug or if the patch notes are wrong.
Dwarven Waraxes are counted as exotic weapons for the purposes of the Child of Faith and Beloved of the Divine feats when Dwarves should treat them as martial weapons.
If all the tiles are already lit when you reach the puzzle room, it has glitched and you will not be able to complete the mandatory objective "Survive the Kobold Gauntlet". You will not be able to finish the quest even after retrieving the dragonshard essence and will need to recall. This was supposed to be fixed in U39 but is still bugged as of U40.

Epic Locus of Vol uses a weapon version of ManslayerManslayer: While wearing this item, your weapon attacks gain the devastating ability to defeat a humanoid opponent with a single decisive blow. Powerful humanoids may resist the vorpal strike and instead take 100 points of damage until they have been sufficiently weakened. effect. It doesn't work on a necklace.
Allowing the NPC party (Coyle and his guards) to follow you along the bridge in the Southern Room can cause him to get stuck on the environment and prevent quest advancement.
These skulls can also be avoided for the majority of the second pass by casting Dimension Door after beating Eternity, instead of exiting via the gate by Yvonne.
Clearing out the Cloud Giant Soldiers ahead of the Warforged Titan can result in him becoming permanently stuck. (This makes sense in that you're watching history; clearing things out yourself changes it.) If the titan gets stuck before the battle with the Stormreaver, you might pull aggroed mobs back to him to re-engage the warforged companions he tends to get stuck behind. If, however, he gets stuck after proceeding up the stairs to the barrier and end boss room, then you will not be able to complete the quest. The best method of preventing the titan from becoming stuck is to always stay behind him, let the titan attract all attention, and do not engage in combat until after the titan is dead.
Hostile creatures casting this spell do not appear to receive any benefit from it. Their stated HP does not change, and tracking actual damage dealt suggests they are not receiving the additional hit points behind the scenes.
The model and guest NPCs are based on a combat NPC, and their "flee screaming in terror" script sometimes fails to activate, leading to them helping you against the artistic onslaught. And dealing some pretty decent damage, to boot.
Soloing quests with a character that currently has enough XP to advance to another level appears to confuse the script in charge of spawning hounds. In some cases, that character may stop attracting hounds altogether.
Behaves weirdly on Legendary - despite being typed as an Ice Mephit, often looks like an Air Mephit, uses Abrasive Breath, and summons Greater Air Mephits.
In a similar fashion to the bugs in Devil Assault, many monsters in this quest count as a different monster in the Monster Manual. For example, any minotaurs in the quest register as a Dock Worker.
Selecting the 2nd dialog option (ending with "Excellent. On my way.") ends the dialogue but doesn't open the door. Speak to the guard again and choose the other option.
Be careful about damaging Goralin too fast on Heroic; if he is reduced from 50% to 0% hit points in under about two seconds, he will die before triggering the next objective and the quest will break.
The ballista has a small chance to drop characters inside the final boss arena. Note, however, that if the final boss is killed before somebody runs onto the stairs outside his arena, the objective to kill him never activates, and the quest cannot be completed. If you end up inside the final arena, it's advised somebody uses Dimension Door and goes to trigger the objective manually.
Vermin type spiders are actually affected by the Blinded and Silenced portions of this spell, although they will not display the visual symbols; however, the effects can still be seen on their examine screen.

If you recast this spell many times while initial buff is on, roughly 25~30% of chance you get +30 seconds to initial buff duration. And seems no limit to increase buff duration.
As a druid in bear form these doors can be passed without Barret's help using the Bear Rush attack, however this will not destroy the door and thus prevents the quest from advancing.
Elixirs actually grant an additional +100% Renown from what is indicated on the potion, so even a "Minor" (+10%) elixir will increase Guild Renown by +110%, or 2.1x the base value.
If you speak to Haverdasher after the door is closed and before it is opened again (because some party member enter late or because you re-entered the challenge), then this will cause the challenge to offer rewards for 1 star less when it is completed.
Hellball is only subject to 1 reflex save. If a mob saves, all damage elements are halved. If they don't save you get the full damage on all elements. Now, when they don't save you get the fire, electric, acid, and cold damage. However, when they do save, you get fire, electric, acid, and sonic damage.

Modius frequently moves into a wall and won't come out so you cannot finish. Surrounding him in the middle of the spa may work, but he has been observed actually teleporting from the middle of the room into a wall.
The description for this spell does not correctly indicate the spell's damage (it claims a value of 2d3+6) nor does it identify that the spell requires a Spell Resistance check.
When the stands collapse, spectators can fail to become active when they fall or may fail to fall at all. When this happens, one person Dimension Dooring and standing just outside the door to the arena area (next to Zeslyn Cay'Vorym's room, not outside the pit cage) will cause bugged spectators to become active until that person moves. The inactive spectators make themselves transparent, so they'll be very difficult to see aside from their eyes. It is also possible that status attacks which incapacitate the lesser worms cause them to burrow and not return; see DDO Forums for more discussion
When a quest is created on DDO's internal software, a single Kobold is placed inside the dungeon by default. This is not always removed, meaning certain quests (such as Through a Mirror Darkly) may include erroneous Kobolds. Kobolds that spawn in this way, for some bizarre reason, appear in the Monster Manual as a Rat called Failed Experiment.
There is a secret door in the room before the Flesh Render which leads to a volcanic area blocked off with a fiery barrier, leading to an optional red-named Invading Fire Reaver Lord. However, this barrier cannot be raised from outside (although it can be bypassed – see below.)
Despite being able to target oneself with this spell, self casting currently (U45) is bugged and has no effect on the caster. Casting this spell on an Ally will properly apply it's effect.

While he always spawn at Bargain of Blood after you defeat the General Manager, he does not appear on your Monster Manual there. To get him listed, you need him to spawn at The Tide Turns.
Hirelings and summoned creatures do not understand the "holodeck" quest scenery, and will likely stand in place doing nothing. Melee hirelings cannot path to enemies, but they may still attack if manually teleported into melee range. Ranged and caster hirelings will attack enemies within range but also try to fire through walls.
The section of the quest where you enter the ship can sometimes momentarily bug out, dropping plot-critical NPC Lucy Lawful through the floor. You have no choice but to recall and reset if this happens.
Probably go slow on Vaspar and careful with damage over time effects, the quest can be rendered incompletable with Vaspar alive with negative HP (not clear why).

Sannyasi has a mechanic that will reset his health bar to full health. Due to a large number of lingering issues with the PvP system, the Death Match and Capture the Flag arenas are currently closed; however, a player can still attempt to travel to them. As you cannot leave a tavern while a match is pending and there is currently no way to cancel the match, this will only get you stuck in the tavern. This can be fixed by closing and restarting the game to forcibly break you out of the match.
If you continue on past the first rest shrine, don't change your mind and go back. The path before you may close, making it impossible to complete the quest.
Five Rings (Slave Lords Crafting) is not a real ring. Reaper Forge may accept your ring crafting recipe and consume your fragments but fail to grant any bonus.
The animation of the fiery explosion triggers immediately on the initial casting of the spell but appears to deal damage when casting animation has finished.
Be cautious of going into the Decaying Battlerager's crypt without Dimension Door; at times, the door doesn't open back up to let you out. (Alternatively, save it for after the end fight, unless going for the kills bonus.)

Do not AoE instant kill or otherwise kill end boss mobs before they become red named after you talk to them and they become hostile, or quest will become unable to be completed.
Update 16 wiped off some Festival Glaciation effects from items created on previous events (possibly just the Glaciation VIII and IX effects). Players must create an in-game help ticket to get a refund. Pre-U16 amount of Motes and Recipes are archived here.
Do NOT accept to repeat the Eastern or Western quests if you want to advance to the third part! Accepting the quests again will require that you repeat them before continuing.
A single Favored Soul level is currently also not granting access. It is presently unclear whether further Favored Soul levels, or post-U51 Divine destinies, will.
The +2 enhancement bonus persists on any weapon held while reincarnating. It can only be removed on a weapon by re-equipping then un-equipping the weapon while you possess this feat.

Adding an Alchemical Eldritch Ritual to a pre-U32 crafted item, can increase its ML by 2 up to ML: 20. This change to ML will not take effect until you log out and in again.
Alchemical Armor and Alchemical Shield are actually coded as item-based Dodge Bonuses to AC, not Alchemical bonuses. The first one of these encountered is a bit buggy - she's supposed to teleport away at low health and is accordingly invincible, but often "forgets" to actually disappear, especially if she gets killed too quickly, and must be kited around for the rest of the quest. She's also always CR 21.
Using a negative energy spell on the Surprise Unicorn breaks its AI, causing it to start attacking like a standard Corrupted Unicorn. This also breaks the location marker on the minimap, so is best avoided.

The quest's two final bosses spawn with different triggers - Thousand Teeth will spawn for the second and final time as soon as puzzle 4 is solved, but Mage Kril won't activate until all four puzzles are solved consecutively. As such, unsolving a tile puzzle allows the bosses to be fought separately.

Temple of the Deathwyrm can become incompletable if players complete the second of two available first floor interior rooms. Players should only complete one of the two rooms in order to prevent the raid from becoming incompletable.
There is a bug where one of the six rooms can't be reached due to a pillar blocking the light puzzle to open that room. This bug is fixed internally, slated for an upcoming patch. It's possible that the real Phylactery was in that room when you destroyed the other five (fake) Phylacteries.

Recent updates have resulted in the Drow-turned-scorpions still roaming about, failing to disappear, after you interact with each to "gather them" into your backpack. They do appear to be seen by the nearby giants, so be mindful after opening the first door if they begin to wander out.
The section of the hallway with Rilvos glitches out if you reach it too fast in the progression and him and some of the nearby Bearded Devils became invincible to all but grazing hits and spell damage.
If certain trials are left until last, the objective to complete all five trials is marked as complete when the fourth trial is complete. If this is the Trial of the Devourer. It still needs to be completed to finish the quest; however, the Trial of the Shadow can currently be skipped entirely.
If 2 pairs of giant and dragon are slain in the same attack, the objective for slaying all of the pairs does not complete; however, the quest progresses as normal. As a result, it is possible to finish the quest (and loot chests) without completing it.

Be careful using pets, hirelings and summoned creatures in this area, especially if above level – if Whisperdoom is somehow killed before a player comes within her line of sight, the optional to defeat her never appears, and completing the quest becomes impossible.
The old flagging system is still in place alongside the new one. Using the old system can break your quest. Avoid talking to Marek Malcanus to prevent the old flagging system from interfering with the current one – he is no longer necessary and really should have had his quest flags deleted to prevent this bug. Additionally, sharing the quest will break it: do not share it or accept any share attempts, pick it up manually.
The Quicken metamagic applied as individually for spell (Through the right-click menu) often causes the spell to miss in many situations. Applied as global not bugged.
In the final room, the large mushrooms can be targeted from outside the room – do not do this! Make sure that you go into boss room to trigger the objective "Destroy large spores"before destroying those, or the quest will not complete after defeating Galatraz and destroying all of large mushrooms!
The Red Musk Infected in this quest are healed by negative energy, and can not have this stripped by pale masters due to being plant typed. Furthermore, since they are typed as living creatures, the Inflict Weariness ability will actually heal them on every hit (and cannot be deactivated without resetting enhancements).
Threads of Fate can behave weirdly if stored in the shared ingredient bank - make sure you have sufficient inventory space to store everything you're removing, or the excess Threads may be deleted.
If you skipped The Salvation of Korthos story arc, talking to Rinar d'Thuranni does not properly advance this quest. This renders the quest uncompletable and causes it to stay in your quest journal forever once bestowed. This is true whether you took the option provided by Jeets, the option provided by Valeria Sinderwind, used Veteran Status to bypass Korthos, or created an Iconic Hero character.
Equipping the Flasks of blessed water may cause bows and crossbows to cease doing damage to anything until you change ammo types and shoot at something.
Since Update 13 patch 2, when the party split up, if you are on the upper area and too close to the bridge when it is dropped down, you will pass through it as if there is no bridge at all. Going back a bit and then crossing it may allow you to pass.
Using Sustaining Song when the rat is about to complete the maze is a guaranteed way to kill your rat after it has exited the maze but before waking up Akhom, rendering the quest unable to be completed.
Rats can get stuck randomly and won't move even if surrounded by Rotten Flesh. If the rat cannot be killed in its current position, you are left with no option but to restart.
One of the dretches associated with the Tower-bound Goristro wave of monsters spawns invisibly on some difficulties before the wave is activated. It can attack you, can't be seen, and takes no damage until you activate that wave by offering the demon's blood to the altar. Once the rest of the mobs of that wave are killed, you can kill the dretch (even though you can't see it) and it is required to complete the wave.
If you do not push west on the way up, it's possible to get stuck against the roof in the reverse-gravity area, and not be able to get down. The "/Stuck" command may help free you.
Epic Trap the Soul effect functions identically to the regular Trap the soul enchantment found on greensteel items (and thus isn't very "Epic"). Despite the release notes and developer posts stating it has a higher hit-dice limit - it currently has NO differences at all. All current testing shows no version of this has any hit-dice limit, and they all share the same ultra low proc-rate, and DC30 will save.
Cixsir's chest only appears after his allies are slain, and he triggers it to be unlocked. Thus if you kill him before you kill his allies, it can not be unlocked. So be sure to kill the other Yugoloth near him before you kill him.
On Monster Manual, it adds as a kill of Warlord Hissyasha (Devil Assault's Heroic Normal boss). If you have Troglodyte Mastery, you can see his HP value.
Blood sacrifice works a little different then the official description which states it only costs 25 hit points. It actually deals 38 untyped damage in exchange for 50 spell points. This damage is affected by dungeon scaling, so while solo you will generally see (roughly as dungeon scaling does vary from class to class) ♣7Solo/Casual♦15Normal♥20Hard♠26Elite●38Epic damage.
Under New Ravagement will fail to complete if you talk to Maxwell too quickly. To avoid this, allow DM to talk before talking to Maxwell and completing.
The specialized randomized items from the Storm Horn Mountains and Wheloon have been updated. These items were incorrectly appearing with Mythic and Reaper bonuses, and were able to be deconstructed in Cannith Crafting. They no longer are compatible with Cannith Crafting. However, they also no longer drop with Reaper or Mythic bonuses, which is not working as intended.
The current setup for these classes is a little unreliable - you are automatically given the effect of a Blood of Dragons on each login, but this expires after an hour. You can reactivate the benefit by logging out and in again.
Under certain conditions, Oksi may refuse to grant his quest chain until you undergo Reincarnation. It is known that this will happen if you teleport to Tanaroa before entering it via Captive of the Hidden God, and then proceed to claim Curses of the Kopru from Mira of the Hawk before speaking to him. It remains unclear if this can happen if you enter Tanaroa via Captive of the Hidden God first, but in the meantime always claim the chain from Oksi before speaking to Mira.
Vorne sometimes fails to spawn, which has been mentioned to be not WAI by the developer. The unlikelihood that this will be fixed is why it's marked as a rare for now.
As of Update 20, if you try to use the "Open all barriers" options, you will always be forced to make a UMD check, even if you had chosen to make a Concentration check before.
Many on-hit damage effects don't work; collars with more passive effects such as Vulnerability, Deadly, Keen, etc. may be a better option. Collars are seen by your pet as accessories not weapons so Red augments that only work for weapons will have no effect in a collar.
Dark Hunters only receive Ranger Quest Rewards with "Quest End Rewards Based on Class" enabled. You may wish to switch this off for a while if looking for a collar.
Be wary of killing this succubus too fast! If reduced to 0HP before her transformation, she will refuse to die after transforming, and will continue to bother you for the rest of the quest.
Both the shortsword and the bite seem to deal slashing damage on some attacks and piercing damage on the rest of the attacks, even though usually weapons either deal one type of physical damage or more than one type at the same time, not alternating.
There is a graphic bug that won't show the red dot from the laser on the floor in the right position. If this occurs just tilt to "watch" the top/ceiling and then back down to the laser point on the floor and it should be in the right position.
For all items with clicky charges except the Litany of the Dead and Purging the Pantheon, the altar claims the items gain more charges than they actually gain (the amount of extra charges you don't get is equal to the base amount).
There is a display bug that blade barrier may render very thin and be difficult to see. If you look really hard, you can still see it faintly as if it is spinning much faster than it should be. This causes no change in its effectiveness, other than a little extra difficulty for the caster to properly space them out so as to not overlap them.
After quest completion, the summary states "You have met Sigmund Bauerson, owner of the Wavecrest Tavern. He is in charge of the local resistance, and has work for you." You do not need to talk to Sigmund Bauerson to complete this quest.
Druid and Artificer characters can not acquire this quest because their trainers do not bestow or act as a contact for any of the Korthos Tutorial Quests.
Pets and quests that are in Wilderness areas do not always interact well. If a pet is "parked" (ordered to "stand ground", i.e. hold position) in a quest, and that quest exits into a Wilderness area, you must order your pet to teleport to you before entering another quest or exiting the wilderness area. Failure to do so will result in losing the pet's command bar, but not being able to re-summon/revive/call the iron defender. Quickly logging out and back in will allow you to summon the pet and continue as normal. Once the pet is not parked, they should follow you as normal.
As of this posting (mid-July, '19), some enhancements work with all non-repeating crossbows, including Great Crossbows. The in-game tooltip and math matches this. Unclear if this is working as intended or not.
If you talk to Scratchy Cranshaw too quickly after he revives, he may not drop key. To avoid this, wait for his name to reappear above his head before talking to him.
Note that this quest WILL allow you to enter Part 3 without having officially completed this part by picking up the map on Markessa's table, thereby forfeiting completion XP and requiring a do-over for the arc-end reward. So, "read" (click) the map on the table or you don't get XP credit for this quest.
It is unclear if this is by working as intended or just an oversight, but upon completion, if you exit via the door, you exit the quest in The Harbor in front of Guard Crichton, the quest giver for Kobold Assault! You'll have to go back to the north Marketplace to get your reward from Loghan d'Deneith. If instead of using the door you choose "Finish", you exit where you entered the quest.
One of the locations for the spider crest, a small pile of stones, will give a "spider crest blocked" message if you are standing directly on/near it. You must stand a short distance from it to take it.
If Victor isn't talking, it's possible one or more of the forgewraiths got "lost" in combat. Go back to the office and all the lower rooms and find them (or let them find you) and kill them. Once all are dead, Victor will be heard to cry "All right, don't do anything rash. I'm coming out. Perhaps we can make a deal...", which signals the end of combat, and speaking to him will end the quest and unseal the end chest.
If an item's ML is raised by a higher ML augment, and then that augment is removed or replaced by a lower level augment, the item's ML will not lower until after the character relogs.
Unless you have spent at least 1 Skill point in Tumble, you will only be able to do single somersaults, despite your total Tumble skill being 10 or more.
When fighting the necromancer at the end, as soon as you kill him, most of the undead in the room will die as well. Therefore, it's usually a good idea for everyone to focus on him first – however, avoid killing him too quickly, as if you kill him in under ~2 seconds, the script that kills all his undead minions will not activate.
At the time of this update, many enchantments were bugged and showed up with a +0 bonus, and some still rarely do (eee "update" comment). This does not appear to affect the item's Minimum Level, for better or worse. Update: Most have been fixed, but some few older reward lists can still produce this +0 bug.
If you lure the spiders off the platform and kill them, the ladder up may not appear. A Jump skill of ~35 can get you to the platform up (climbable spot is somewhere on the left side of the platform when you first face it
Bags will not automatically accept new multiple items if the number would exceed their capacity. E.G. Your Tiny Ingredients Bag has a max capacity of 1,000 of any item. If it currently holds 950 of an item, and you collect another 51(+), those will go into inventory and not into your bag. Manually "gathering" will collect as many as the bag can hold up to its max, leaving the remainder loose in Inventory.
The Dungeon Master speaks of the path forward being "...through the open doorway to the west." In fact, the door out of the main entry room is north, on the ground floor, closer to the east end of that wall, then west down that northern hallway.
Avoid bursting down the dragon too quickly (Ruin, Manyshot, etc.) Goralin is supposed to walk away from the first encounter once down to 50% health, but if you kill him so fast that he dies before turning passive, the quest may glitch and make it impossible to progress.
During the Trial of Endurance, a Shavarath Hezrou has the potential to spawn or get stuck inside a wall, with no way to deal damage to it, softlocking you in the dungeon, as you need to eliminate every enemy in order to despawn the portals to advance. May be completable if that happens, after the lever has been pulled.
It is unknown exactly how, but it is possible for the third seer to be "dislodged" from the crystal. He will slide over to the mouth of the ramp the Minotaurs arrive from and will continue the ritual there. This does not affect the ritual in any way, though it does make it slightly harder to protect against the Minotaurs, since the seer is closer to them and one will frequently take a shortcut and slide up the steep slope. This seems to happen most consistently when a solo player falls off the plateau.

Official description states an incorrect damage cap and maximum caster level. The standard "spell properties" section of the tooltip states that there is no maximum caster level, and in-game damage seems to confirm that the spell scales at least as high as CL 20 (about 640 damage per tick with 221 force spellpower, so 200 base damage per tick, very close to the expected average for 20d6+120 which is 190).
Description for Quality Armor Bonus reads armor bonus to AC, but is in fact a quality bonus to AC (categorized as "Misc" in Armor Class breakdown in character sheet).
This monster's behaviour in the Monster Manual is rather unusual - while it counts as a Fire Giant, it will often not actually appear as a kill in the Monster Manual until another type of Fire Giant is slain and the count increments, at which point every Fire Giant Champion slain beforehand will appear at once.
For all items with clicky charges except the Litany of the Dead and Purging the Pantheon, the altar claims the items gain more charges than they actually gain (the amount of extra charges you don't get is equal to the base amount).
When both parties are standing on an island and one of you is going to cast an island next, try to make sure you do not stand on or behind the player casting the island. Otherwise, being too close can cause the DDO aim mechanic to fire the wand charge in a random direction. You could fail the challenge as you have no island to progress to when the one you're standing on dissipates.
Appears to always spawn with its Normal statline. This and the fact it is missing from the Monster Manual suggest it is summoned by the Rusty Iron Kitchen Key.
You can see Dwalni Demonsbane's HP if, and only if, you have unlocked the ability to see Ghoul HP counts from their Monster Manual entry. However, Dwalni himself does not count for the Ghoul MM.
