This item can be found in the Level 11 quest The Spawn of Whisperdoom. Once outside the quest, the Ungurz Lieutenant will exchange the Splinterskull Historical Documents for your choice from a short list of randomly generated items - the Widowblight sickle has a rare chance to be in that list.
