I think the below item is it, but I'm not sure. The '+5' part of the name seems to have gone along with the 'Improved III' nomenclature, and so the page name should be changed to avoid confusion. The CL is the same at 5, which is why I believe this is the same item. But I don't want to replace the main page image without giving some opportunity for comment.

- Eternal Wand of Ray of Frost exists already. I'm not sure where/how the changes were applied. I'd set this aside and make a new one from scratch this winter and document it right. - Eternal Wand of Ray of Frost exists, but it is not the same item despite the similarity of the name. The item you linked has a CL of 2, while the item I uploaded the image of has a CL of 5.
