there is a portal missing on the map, in the northwest corner of the Halls of Lunacy chamber, it leads to a platform high on the wall in the alcove on the North of the Devil area (east of the entrance to VOD) that is otherwise inaccessable -- anonymous user, 03:13, December 3, 2012

There seems to be an issue with 2 of the rares. On the map, Orthon Nuthdronu also appears at the location marked for Sorceror Tarethru. In fact in 30+ runs I have not been able to get Tarethru to appear at the location shown, although Nuthdronu has appeared at both numerous times.
