It seems that Skull Grenade is the DDO Store name for Skull Heads. The name, per the naming policy, should be the name of the item itself in-game for the description window. After doing a page comparison, I can see that Skull Head is the more complete/correct version of what the page should be, but Skull Grenade does actually have some information that Skull Head does not.
