Ogre Khan's Skull can be found in Bring Me the Head of Ghola-Fan! after Ghola-Fan is defeated. Go left from Ghola-Fan's throne room and check the bone pile. Ogre Khan's Skull is used in the underground passages in the Restless Isles to reach the Volcano's Mouth the location of The Twilight Forge.
