Monster list: missing a variety of Erinyes (Heroic Elite), but I didn't take note of their exact title or level.
Codex Runes[edit]
Still getting used to the templates/dependencies on these pages; it looks like I can't directly edit anything in the "Named chest loot" section, but: from my (limited) experience, it appears to be 3/7/9 runes on LN/LH/LE. I probably still have the other screencaps if needed. Creeping Death is also 3/7 for LN/LH, but I won't be trying LE again any time soon as my character very much cannot handle it yet.
