To obtain the quest and a Shield: Frame talk to Racella Unary at the Upper Necropolis The Necropolis.
Gather the 8 different Shield Fragments through four required quests of Litany of the Dead Part 4 and turn in a completed Shield: Frame to Racella Unary at the Upper Necropolis The Necropolis, to receive a shield from the reward list.
As-of Update 13, Shield Fragments can drop in most chests in all 4 Necropolis IV quests, and rare chests in the Orchard of the Macabre. But, specific fragments are most common in specific chests as listed here.
The numbers are the positions of the piece in Shield frame (left to right).

• Shield: Fragment One (1) and Shield: Fragment Two (2) drops mainly at Desecrated Temple of Vol (Marut chest).
• Shield: Fragment Three (3) and Shield: Fragment Four (4) drops mainly at Ghosts of Perdition (Undead wave chest).
• Shield: Fragment Five (5) and Shield: Fragment Six (6) drops mainly at Flesh Maker's Laboratory (Golem wave chest).
• Shield: Fragment Seven (7) and Shield: Fragment Eight (8) drops mainly at Inferno of the Damned (Locked Acolyte chest).
