Mythic tier of named items have been introduced in Update 23: The Mark of Death. . Original form of this mechanic only continued for Update 24: Heart of Madness. It was short-lived and has been replaced with Mythic Boost variants in Update 25.

Mythic items are simply much rarer, stronger versions of certain pieces of loot. They don't only drop on Elite, but the difference in drop rate between N/H/E is much greater for Mythic items than for other items; your best bet for farming one is to run Elite.
